The Effect of Intraarticular Injection of Hyaluronidase in Patients with Adhesive Capsulitis of Shoulder

Abstract
Objective: To evaluate the efficacy of intraarticular injection of hyaluronidase in adhesive capsulitis of the shoulder.

Materials and Methods: Forty patients with adhesive capsulitis of the shoulder were assigned at random to receive intraarticular injection into the glenohumoral joint. Twenty patients were injected with hyaluronidase 1 ml, triamcinolone 1 ml and normal saline 8 ml (hyaluronidase group). Another 20 patients were treated with triamcinolone 1 ml and normal saline 9 ml (control group). The same physical therapy program was carried out for all patients. We examined the range of motion (ROM) of the shoulder, Visual Analogue Scale (VAS) and Shoulder Pain and Disability Index (SPADI) prior to and 2 weeks after injection.

Results: The ROM of the shoulder for both groups all improved at 2 weeks after injection. However, the hyaluronidase group showed significantly greater improvement than the control group in abduction and external rotation (p£¼0.05). The SPADI score had improved significantly more in the hyaluronidase group than the control group (p£¼0.05). VAS of the two groups were improved significantly at 2 weeks after injection. But, there was no significant difference between them.

Conclusion: Intraarticular injection of hyaluronidase would be a good treatment method in improving ROM and disability of the shoulder in patients with adhesive capsulitis.
